Syringe aspiration rather than traditional liposuction is considered a less traumatising and safe technique for harvesting fat for grafting (or for small quantities of fat). It is not always possible to get large quantities of fat grafts using this method. It is still controversial as to the size of cannulas or syringes to be used for aspirating fat to harvest grafts. A recent study shows that it is better to use a 2 mm diameter cannula with a blunt tip and several small holes connected to a 10 cc syringe to harvest fat grafts. Using a smaller syringe (10 cc) is advisable to ensure minimal pressure is applied during harvesting.

Using a Luer-Lok syringe that delivers a 10 cc syringe to a cannula. We prefer to use 10 cc syringes instead of larger ones, because smaller syringes are less cumbersome to hold in the hand. We use a cannula that is 15 cm long and has a blunt tip, similar to a bucket handle. It has multiple openings on both ends, similar to the way bucket handles have dual openings. Pulling back gently on the plunger enables us to create a 2 cc negative pressure on the syringe. After harvesting fat, incisions are closed with sutures that are separated from the surrounding skin. It is safe to harvest fat grafts with this method for transplants with a volume less than 100 cc.

If you have very large amounts of fat in your body, this low-pressure method produces a higher viable adipocyte count than if you use high-pressure aspiration (600 mm Hg).
